The defence letter is intended to provide the client with a holistic package of guidance and access to medical services.

The letter of defence grants the client the "realization of medical rights" both financially and in other areas that will be detailed below.

Support and guidance to the client in receiving assistance in handling a range of problems: financial, family and bureaucratic, with the aim of exhausting all of the client's rights and needs.

This letter of defence between "Life and Protection Realization of Rights Ltd." and the client provides a package of "supporting services" to ensure the client's rights are fully exercised.

This defence provides real assistance in managing the client's needs and exercising their rights against various parties.

The defence includes:

Accompanying the client by a company representative who performs a feasibility study through analysis of medical findings and building a claim file to exercise his rights against various entities, government ministries and others.

Long-term care insurance claim

Car accident claim

Fall accident claim

Protected recreation claim

Income tax exemption claims

Volunteer claims

Private insurance claims for personal accidents and/or disability from an accident

Critical illness insurance claim

Assistance in locating medical experts to complete the missing medical information in the file

Eligibility check and processing of issuing a disability certificate

Assistance and guidance in exercising rights with government ministries and public bodies 

Assistance and guidance in exercising rights within the framework of an insurance claim

Cell phone theft - In the event of a cell phone theft, the company will charge the customer compensation of

Up to 250 EUR or more if the customer has contents insurance through one of the companies, the company will help and handle the claim against one of the companies to insure the compensation for the customer (one-time compensation, in which case the section in the service letter is canceled) for the purchase of a new device upon presentation of a report from the Israel Police and an invoice that a new mobile phone was indeed purchased from one of the authorized resellers. The company works with all stores in Israel.

The service can only be claimed after 3 months of training.

The company has connections to most stores in Israel, so it also promises to help you find a device at the cheapest price.

And make sure to maximize these 250 EUR for the benefit of the client.

Qualification Period - A period that begins on the date of joining the service. During this period, a covered owner or beneficiary will not be entitled to benefits.

 

The defense letter of life and defenses of exercising rights
The customer will be entitled to receive the services included in the warranty, subject to the conditions and restrictions detailed below.

The services provided under this Service Letter are provided by the service provider, which is not the Company, and are provided under the terms and limitations of this Service Letter.

 

Settings

In this defense, the following terms shall have the meanings stated next to them:

"The Company" - Life and Defenses, Rights Realization Ltd.

"Provider" or "Service Provider" - Life and Protections Rights Realization Ltd.

3.1 "Customer" - a person whose name is stated as the person for whom this Statement of Defense was purchased.

"Defense Statement" – this Defense Statement with all its amendments/expansion as they may be, if any, from time to time.

"Website" – the Supplier's website  www.life-pro.co.il

"The Effective Date" – the effective date of this Statement of Defense, as appears on the insurance details page.

"Qualification Period" – a continuous period of 90 days that begins for each customer on the determining date.

The customer is not entitled to services under this warranty in the event that the entitlement to the service arises during the qualification period. For the avoidance of doubt, it is clarified that the customer will be charged during the qualification period.

The qualification period will apply to each customer only once as long as this warranty is valid.

"Specialist Doctor" – a doctor who is authorized to work in Israel, as a specialist in a particular medical field in accordance with the provisions of any law and the competent authorities, and whose area of ​​expertise is relevant to the provision of services under this Statement of Defense.

 9.1 "Lawyer" – a person who is legally qualified and holds a valid license to practice law in Israel according to any law.

12.1 "Service Center" – a telephone center operated by the Supplier for the purpose of providing service according to this Statement of Defense, operating Sunday-Thursday between the hours of: 5:00 PM-9:00 AM at: 03-6323733.

13.1 "Personal Manager for the Customer's Case" – an employee of the Supplier whose job is to manage all of the services provided in this Statement of Defense.

"Monthly Payment" - Amounts that the customer is obligated to pay as a condition of receiving the services, which are detailed in the statement of defense.

"Client's age" – the age of the client for the purposes of this policy, which is determined as follows: The insured's age on the determining date is determined according to the difference between the client's date of birth and the determining date.

The client's age will be calculated in whole years, with any fraction of a year less than half a year rounded down and any fraction of a year equal to or greater than half a year rounded up. The payment remains fixed throughout the life of the policy from the age of 21.

2. Eligibility for service
For the avoidance of doubt, it is clarified that the customer will not be entitled to receive any of the services detailed in this letter of defense due to a medical incident that occurred during the training period.

3. Details of services
3.1 During the medical case, the customer will be entitled to receive from the provider support, advice and guidance on a variety of topics as detailed below, with the aim of assisting the customer.

 

3.2 Scope of Services:

3.2.1 A conversation with a representative from the company to examine feasibility. If the coverage holder is found suitable, a process of analyzing and building the case for exercising rights will begin:

The case manager will provide the client with an initial explanation of the process, review the medical information, and formulate a summary of action on all relevant areas. The client will be required to act on the summary of action that will determine and complete the required information, with the goal of bringing the process to a conclusion as quickly and successfully as possible.

In accordance with the summary of the action, the case manager will coordinate consultation meetings for the insured with the relevant parties on behalf of the provider, including a company representative and/or others as needed.

 

3.2.2 Without prejudice to the generality of the above, the Supplier will operate in the areas detailed below:

Locating medical experts:

Assistance in locating medical experts to complete the medical file (experts who are in agreement with insurance companies, according to the coverage content of the client's health policy).

Exercise of rights vis-à-vis government ministries and public bodies:

The customer will be entitled to receive assistance and guidance from the supplier regarding the exercise of rights from government agencies, including:

Long-term care insurance claim - private insurance or through a health insurance fund - limited to 5 or 6 years of pension

A nursing care claim through National Insurance - provides help in hours or divided into hours and an allowance according to the score determined by National Insurance.

Car accident claim - for private vehicles and/or bus passengers and/or pedestrians

Fall Accident Claim - Supermarkets, Rough Sidewalks, Wet Floors

Protected Entertainment Claim – Receives full protection within the event without additional charges

Private insurance for personal accidents and/or accidental disability in the event of an accident.

Critical illness insurance claims 

Funding for a disability assessment with qualified experts, in the event of a claim only

Disability Certificate - Eligibility test for heart/lung patients/lower limb disabilities/severe back problems

Handling the issuance of a disability certificate at no additional cost to customers who are in the process of an active claim or for a nominal fee in the event of eligibility.

Up to 12 treatments per year in salt rooms nationwide, for asthma and respiratory problems, subject to a deductible of NIS 125 per treatment.

Up to 12 treatments per year in oxygen chambers nationwide, for a variety of medical problems, subject to self-participation:

First 6 treatments - 40 EUR per treatment, and an additional 6 treatments - 65 EYR per treatment.

Assistance in locating medical experts to complete the missing medical information in the file

Assistance and guidance in exercising rights within the framework of an insurance claim

Examination of the insurance portfolio and its discount - a thorough examination of the insurance portfolio of the policyholder and his family and reflection of the current situation, providing exclusive discounts for existing coverage - up to 30%.

 

Limitation of the scope of services:

It is clarified that the services under this warranty include treatment, assistance, advice and guidance but do not include coverage of the client's expenses associated with receiving them. Payment for these professional services will be borne by the client, paid by him and under his responsibility. That is to say: completion of tests, medical documents, opinions and the like are not in the service document of the provider and will be borne by the recipient of the service. 

 

The service does not include performing actions on behalf of the client before the various entities if a signed power of attorney/confidentiality waiver cannot be provided by the client allowing this.

 

For the avoidance of doubt, it is hereby stipulated and clarified that the services of the Provider and/or anyone on its behalf under this Statement of Defense are not a substitute for medical advice and/or medical treatment and/or legal advice provided to the Insured.

 

The success of the service is based primarily on a feasibility check of the client by Life and Rights Enforcement Ltd. before filing the claim.

 

It is clarified that the Supplier and the Company or anyone on their behalf will not be liable for any act or omission of any of the professional advisors recommended by the Supplier and selected by the Customer. The Supplier and the Company and/or anyone on their behalf will not be liable for any direct and/or indirect damage and/or financial and/or other loss incurred by the Customer and/or any other person in connection with the services provided to the Customer pursuant to this Statement of Defense, including in connection with the Customer's choice of any of the professional advisors/institutes recommended by the Supplier and/or anyone on its behalf.

 

It is clarified that in the event that the Supplier is damaged due to a cause beyond the Supplier's control, such as: fire, explosion, war, the Supplier and the Company will not be responsible for failure to meet the schedules detailed in this letter of defense.

 

5. How to receive services in the service letter
The receipt of services by the customer according to this statement of defense will be as follows:

5.1 The customer entitled to receive the service will contact the provider via the service center, stating: his name, address, ID number and a telephone number where he can be reached. If necessary, he will complete additional details related to the provision of the requested service and the case in which the customer requires the service.

 

5.2 A service representative on behalf of the company will contact the customer and will continue to investigate the request, including the feasibility of filing a claim over the phone.

 

5.3 If the customer is found suitable, the company representative will collect all the data required to map the impairment sections, disability percentages, complete medical information details, including opinions, tests, etc.

 

6. Privacy protection
It is clarified to the members of the association that the security of the information stored at their endpoint is their sole responsibility and that the supplier or service provider has no ability to secure this information. It is the customer's sole responsibility to ensure that their privacy is maintained with respect to their personal medical information, which may be exposed to people in the customer's environment or to other users on the same communications network.

 

7. Receiving services during military service
During military service, reserve or permanent service, the customer is subject to the military's instructions and guidelines, which change from time to time and may limit and/or prevent the customer from receiving medical opinions or tests at private institutes outside the military framework during his military service. A situation that may affect the exercise of the medical rights due to the customer in accordance with the terms of this statement of defense. Information regarding the military's instructions and guidelines, which change from time to time, as stated above, is available from the military authorities. In any case in which the customer is in a military framework, he must be informed about the existence of these instructions and guidelines.

 

8. Client fees
8.1 The payment amount is set out on the details page of this defense.

8.2 The customer will pay "Life and Defense Ltd." a monthly payment. For the avoidance of doubt, payment will begin on the date of joining the defense, even during the qualification period.
 

9. Change in service fees and terms
9.1 The service fee at the beginning of the service is fixed and does not change with age, except for adults over the age of

21. In this case, the customer's fee is updated and then remains fixed, provided that the customer has not canceled the service.

10. Validity of the service document
10.1 This statement of defense will enter into force as of the determining date, without derogating from the qualification period mentioned in section 1.7 above.

10.2 The validity of this statement of defense will automatically expire on the earliest of the following dates:

10.2.1 In any case in which the Company cancels the letter of defense in accordance with the provisions of any law, including for failure to pay the subscription fee in full.

10.2.2 In the event of termination or termination of the agreement between the Company and the Supplier, to the extent that the Company has not reached an arrangement with an alternative supplier.

10.3 For the avoidance of doubt, it is clarified that upon the expiration, cancellation or termination of this letter, for any reason, the customer's entitlement to receive the services under this letter will end. It is clarified that if the customer has applied for services prior to the expiration of this letter of defense, the customer is entitled to continue receiving the service for which the application was made until the end of the provision of services in connection with that matter.

 

10.4 The customer may at any time cancel this warranty by written notice to the company. In this case, the cancellation of the warranty will take effect on the date the company receives the notice. The customer will be entitled to a proportional refund of the price of the warranty paid, except in relation to a customer who continues to receive services after the end of the period.

The letter of defense, as detailed above, and which will continue to be paid until the end of the month in which the service was last received.

 

10.5 This letter of defense is canceled, for whatever reason the company will cease to collect payment from the customer for the letter of defense.

 

11. Linking conditions
11.1 All amounts stated in the defense, including the monthly payment, deductible amounts and maximum indemnity amounts, will be index-linked and updated in accordance with the ratio of the new index to the base index.

11.2 "The Index" means the Consumer Price Index, including fruits and vegetables, published by the Central Bureau of Statistics from time to time, including the same index even if it is published by another official body or institution that replaces it, whether or not it is based on the same data on which the existing index is based.

11.3 "Base Index" The September index published on 10/15/20, which stands at 12,340 points.

11.4 The "new index" will be the index known on the date on which the relevant payment was to be made.

12. Miscellaneous
12.1 Any dispute that may arise between the parties in connection with this defense will be heard exclusively by the Tel Aviv court.

12.2 It is clarified that the right to receive the service according to this statement of defense is personal and non-transferable to another.

12.3 The address that appears in the Company's records as the Customer's address is the binding address for the purposes of this Service Letter. If several subscribers entitled to this Service Letter are listed in the Statement of Defense, then notices sent to the main customer, as defined by the Company according to his last address provided to the Company, will be considered notices delivered to all members of his family.

12.4 Any notice and/or declaration to the Company and/or anyone on its behalf will be delivered in writing to the Company's offices.

12.5 Everything stated in this defense in the singular also means in the plural, and everything stated in the masculine also means in the feminine, unless expressly stated otherwise.

12.6 The section headings are for convenience only and shall not be used for interpretation.
